High Altitude Tune for 7.3 + Other Questions!

I need some help please!

Revision: 7R19DZ ? I think. (couldn't update to the newest version)
Vehicle: 2003 Ford F250 7.3 D

I installed the 60hp performance tune and the truck runs so much better. I also put a firm shift tune on the truck to make the shift points better. I was able to increase my mileage from 13mpg to 21mpg in town! I haven't had any problems with trouble codes.

So here's my questions:

-Should I put a high altitude tune on the pickup as well since we are located at nearly 6000ft? If I do need to apply this tune, do I have to take it off before going into lower altitudes?
-How do I change the transmission type? and is it necessary?
- The truck has a 4" lift, so do I need to adjust the tire size? if so, how?
