My own fault - dealer flashed me

2008 Dodge Charger RT with Road & Track 5.7 Hemi - U7135

I forgot to restore before going to the dealer... hadn't touched the tuner for years but still no excuse.

I think I get the gist of the hoops to jump through, please confirm/correct/append:

1. Provide proof from the dealer - please let me know exactly what is needed and how to get it to you.
2. Power up and call?
