VXAP2N2 R17 - still prob. on 100 hp. setting

Customer w/ an 04 6.0 Cal ID VXAP2N2, version R17, still having surging issues on the 100 hp. setting. Lower it down to the 60hp & NO PROBLEMS. Fuel Filter was changed less than 500 miles ago. Any ideas?
