i have a 2003 f150 with 5.4L. What does turning off the rear O2 sensors do? will it affect how my truck runs what is the point of having that option since there is a good chance that 90% of people using the tuner are not racing off road only.

All it does is keeo the CEL from coming on. Does not affect how the truck runs.

So if you removed the cats and then turned off the rear O2 sensors, would it pass or fail emissions? on a 2003 OBDII equipped truck? since the sensors are turned off and not throwing a CEL.

It would not pass emissions like that at all.
