Reprogramming my Delta Chip

I have an 89 5.0 LX that I'm putting a 347 Stroker in. I have the Delta chip that was burnt for my previous mods and I need a new flash for this setup. My question is, what info do you need and how in-depth do I need to go on my mods, and where do I need to send it off? Thanks.

I talked to my local dealer and he said I should contact Diablo directly by filling out some custom tune form and e-mail it to Diablo . Since I have nothing stock on the engine and all the different variables involved between what I have and what they can get and flash on the chip. This way I can have a truly custom chip. I looked on the site and didn't find the form he was talking about.
