All I got was the 87 octane tune and the 93 octane tune. Isn't it supposed to come with a 91 octane tune? 93 octane isn't available here in Utah.

I am pretty sure you need to remove 2 degrees from the wide open throttle timing menu which I believe is in percentages no degrees.
