01 7.3 Powerstroke hunting for gears and studdering

I have an 01 7.3 with 4" exhaust, cold air intake, and the Diablo 1770 tuner. I have noticed that when I am rolling at about 25 mph and then stand on the throttle, the truck will hunt for gears and then take off.

Also, cruising at 40 - 70 the truck feels like its hesitating. All the gauges are showing good levels.

All of this happens with both the 85 and 110 tunes. I have the tuner updated to the latest CROM. Any suggestions?

stock sized tires?
